1. ASSUMPTION OF RISK: I hereby acknowledge and agree that participation in fitness challenges, workouts, and events organized by or associated with AZFitClub involves inherent risks of physical injury, illness, or death. I assume all risks associated with my participation.
2. RELEASE AND WAIVER: I, for myself, my heirs, and legal representatives, hereby RELEASE, WAIVE, DISCHARGE, and COVENANT NOT TO SUE AZFitClub, its owners, organizers, affiliates, and partners from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me.
3. MEDICAL CONSENT: I attest that I am physically fit and have sufficiently prepared for participation in these activities. I have been advised to consult with a physician before beginning any exercise program.
4. PHOTOGRAPHY RELEASE: I grant AZFitClub permission to use photographs, motion pictures, recordings, or any other record of this event for any legitimate purpose.
By checking the box during registration, you acknowledge that you have read and understood this Waiver and freely and voluntarily agree to its terms.
